CorEVISION SCANNING GPR RELEASE OF LIABILITY/AUTHORIZATION

In consideration for the opportunity to receive GPR(Ground Penetrating Radar) concrete scanning services from CoreVision Scanning GPR (“CoreVision.”), |. be undersigned “Releasor’), as behalf of yself and legal representatives, heirs, ar-assigns, hereby release, waive, and discharge waive, and disclaimer from any all liability for damages, loss, or injuries. The Releasor understands and acknowledges that the depth and accuracy ofGPR scanning are dependent on the electrical properties of the material being scan- ned and other environmental conditions. Interpretations of radar data are ba-sed on professional judgment. CoreVision does not guarantee the accuracy or ¢ completeness of the scan results. Ifany provision ofthis agreement is decemvrisk of loss or damage to persons or property based on the data provided and agrees that CoreVision is not liable for any resulting consequences is the current job and all future services.If any provision of this agreement is deemed invalid under applicable laws,the remainder shall stay in full legal force and effect.CoreVision is not responsible for delays or josses from data misinterpretationre- liance. on scan results,or improper preparation ofthe scan area. Additional char- ges of $200:00 per hour per technician apply if the job exceeds standard sco- pe or time due to inadequate site preparation or extended coverage. Each job has a minimum charge of2hours at $400.00. After-hours and weekend work may be subject to higher rates Cancellation Policy: Requires 24 business-hours notice.
